Adenotomy – removal of the pharyngeal tonsil

Adenotomy involves the truncation of a mass of hypertrophied tonsil. It is recommended that the procedure is performed under general anaesthesia, which ensures the safety of the child, eliminates their stress and ensures the precision of the procedure. The pharyngeal tonsil (known as the third tonsil) is situated in the nasal part of the throat. The oropharyngeal tonsil is not visible on examination through the mouth, it is located at the back of the nose and can be seen through the nose or on mirror examination.

Adenotomy – distressing symptoms and indications for surgery 

If a child breathes frequently through his mouth, has a constantly open mouth and a somewhat staring expression, and snores or breathes loudly at night, he will wake up tired in the morning. During the day he can be lethargic. He sometimes loses his appetite. Prolonged rhinitis, recurrent otitis and hearing loss occur. The voice becomes nasal, as if it has a noodle in its mouth.The most common culprit is an oversized third pharyngeal tonsil. Such hypertrophy narrows the patency of the nose and upper airways. It also impairs the function of the ear trumpets, which promotes effusions and otitis media. There are also problems within the paranasal sinuses, sometimes in a specific sinus, such as the maxillary sinus. Children’s tonsils often enlarge as a result of bacterial or viral infections . 
If pharmacotherapy is not effective, the ENT surgeon may suggest surgery. The aim is to remove the hypertrophied tonsil. The procedure is usually performed under general anaesthesia, although the decision on anaesthesia is made on an individual basis.
The post-operative period is usually short. Painkillers and a calm daytime routine are needed. Minor bleeding is possible, which usually subsides quickly.

Contraindications to adenotomy surgery 

Adenotomy is postponed or discouraged in several situations. During the ENT consultation, the doctor takes a history to determine whether such contraindications exist.

Although conservative treatment can sometimes be effective in relieving symptoms, it is important to see a specialist if you experience any breathing difficulties, persistent infections or other worrying symptoms. Post-operative management – recommendations after surgery

For safety reasons, the patient stays on the ward. Immediately after the operation, you cannot drink or eat. For the next 5-7 days, physical exertion and the consumption of hot and irritating foods is not advisable. It is advisable to rest and limit contact with others to reduce the risk of disease
