Indications for FESS surgery – when is surgical treatment necessary?

FESS (Functional Endoscopic Sinus Surgery), or functional endoscopic sinus surgery, is a modern and minimally invasive method of treating chronic sinus conditions. The indications for the procedure are primarily chronic inflammation of the paranasal sinuses that is not sensitive to drug treatment, i.e. that persists for at least 12 weeks despite the use of intranasal steroids, rinses and antibiotic therapy. FESS surgery is also used for recurrent acute sinus infections, nasal polyps, mucormycosis, fungal sinus infections or anatomical abnormalities that impede proper sinus ventilation (e.g. curvature of the nasal septum, concha bullosa). Other indications are the presence of cysts, benign sinus tumours and the need to remove foreign bodies or repair cerebrospinal fluid leaks. FESS surgery restores the patency of the natural sinus outlets and improves mucosal function, and the decision to perform it should always be preceded by a thorough diagnosis and a trial of endoscopic sinus cleaning.

What does endoscopic FESS surgery involve?

Endoscopic sinus surgery using a microscope and an endoscope allows, in a minimally invasive manner, the dilatation of the sinus orifices so that secretions can escape freely. Endoscopic treatment can stop the process of mucosal hypertrophy and thus protect us from more serious complications. Surgery performed in this way offers the chance to improve the condition of the sinuses, while leaving no visible scars as it is performed inside the nose. The procedure aims to restore normal nasal cavity patency.
Surgical instruments are inserted through the nasal orifices. The surgeon uses a surgical endoscope and can thus follow the image on the monitor. Special forceps are used to widen the natural connections between the paranasal sinuses and the nasal cavities. Through this, the function of the sinuses, their natural clearing of mucus and ventilation is improved. It is a much more sensitive method than those previously used.
