Penile prosthesis implantation procedure – treatment of erectile dysfunction

The AMS 700™ and Tactra™ penile prostheses are designed for men suffering from chronic, organic erectile dysfunction that has not responded to other erectile dysfunction treatments or injection therapy. Each penile prosthesis provides a natural appearance, adequate rigidity , discretion and a high patient satisfaction rate of more than 95 %. At the Medicus Clinic in Wrocław, we decided to implant products made by the American company Boston Scientific, because the AMS 700™ hydraulic penile implant is the best tested solution on the market, has the highest patient satisfaction rate and has been improved with a special Inhibizone™ coating, which reduces the risk of infection and the possible need for subsequent implantation (risk of approx. 2-3%).
The manufacturer provides a five-year guarantee against possible mechanical damage to the penile implant , and thanks to close cooperation with distributor Boston Scientific, it has been possible to significantly reduce the cost of the procedure, making this type of surgery affordable for far more of our patients.
Men are the most common patients opting for penile prosthesis implantation:

with disorders of organic origin (irreversible vascular or nerve damage),
after surgery for the radical removal of the prostate (so-called radical prostatectomy), radical removal of the urinary bladder (so-called radical cystectomy) or surgical excision of the rectum,
After spinal cord and pelvic injuries,
with long-term vascular complications in the course of diabetes.

Among the available surgical treatments for erectile dysfunction, the following are used:

hydraulic penile prostheses , where an erection is obtained by squeezing a pump hidden in the scrotal sac several times ,
semi-rigid penile prostheses, where an erection is achieved by bending the rigid penis away from the body surface.

Types of penile prosthesis – hydraulic or semi-rigid prosthesis – find out the difference

AMS 700 ™ hydraulic penile prosthesis – a prosthesis consisting of three components: a fluid reservoir concealed in the abdominal cavity, a pump in the scrotum and cylinders in the penile corpora cavernosa . It is this last component of the penile prosthesis that is responsible for producing an erection , but this only happens after active pumping of fluid from the reservoir previously located in the abdomen. At the end of sexual activity , press the button on the surface of the pump and squeeze the penis vigorously, thus moving the fluid back into the reservoir.
The surface of the prosthesis is coated with a special antimicrobial layer, Inhibizone™, which reduces the risk of infection and the eventual need to remove an infected implant .
Semi-rigid Tactra™ prosthesis – penis implant , whose construction consists of flexible nitinol wires surrounded by two-layer silicone. This design ensures durability, ease of implantation of the prosthesis and a very low risk of possible mechanical failure. The process of getting an erection is simple: you simply bend the penis upwards. On the other hand, if you want to achieve a resting state, you bend them back down.

Urology consultation

If you are struggling with erectile dysfunction and other methods have failed you – arrange a consultation with our specialists.

Patient qualification process for penile implant

During the qualifying visit, it is crucial to take a full general and sexual history , rule out a psychogenic aetiology of erectile dysfunction and clinically assess penile anatomy . It is also very important to know the expectations of the patient himself , as well as his regular sexual partner – hence her presence during the pre-operative visit is welcome. If there are no contraindications to the procedure, the patient’s consulting urologist will order the tests necessary for a safe implantation .

Make an appointment for a consultation

If you are interested, make an appointment for a qualifying visit, which will be conducted in Medicus Clinic by dr n. med. Adrian Czekaj , urologist and clinical andrologist.

After a two-day stay in the clinic, the patient goes home or to a hotel, where he or she has the opportunity to interact with the medical and nursing staff for a few days. On the third day after surgery, an examination takes place at the Medicus Clinic outpatient clinic to assess the occurrence of any complications.

Typical symptoms after the procedure are swelling and bruising of the penis , perineum and scrotum, which can last up to several weeks. During the operation, a catheter is often inserted into the urethra and bladder, usually removed the following morning. The penis and scrotum are wrapped in a tight bandage to prevent the formation of a haematoma.

The patient is sent home or to a hotel with an information sheet with recommendations:

use of an oral antibiotic for 7 days,
The dressing should be removed after 48 hours, the wound washed with antibacterial soap,
in the first 2-4 weeks, avoid alcohol, lifting, heavy traffic and wetting the wound,
After 2 weeks, daily activation of the implant is recommended – for AMS 700™: pumping for 15-30 minutes, for Tactra™: bending and flexing,
sexual activity should be avoided for a minimum of 6 weeks,
Urological or email follow-up takes place after 1, 3, 6 and 12 months.

Contact your doctor immediately if they occur:

pain that does not go away after painkillers,
Increasing swelling, pain, bruising persisting for more than 2-3 weeks,
in the first 2-4 weeks, avoid alcohol, lifting, heavy traffic and wetting the wound,
loosening of sutures or opening of the wound,
Urinary tract infection symptoms: burning, frequent urination, blood in the urine,
symptoms of general infection: increased pain, redness, pus, fever, inability to urinate.
