Tonsillectomy is a procedure involving the complete removal of the palatine tonsils. In younger patients, however, partial excision, or tonsillotomy, is often chosen, which reduces the volume of the tissue while leaving a piece of it capable of performing its immune function.

Indications for tonsil surgery in adults

Recurrent inflammation and tonsillitis, as well as chronic inflammation of the palatine tonsils and breathing difficulties are the main reasons to consider tonsillectomy in adults. Hypertrophy of the tonsils results in the accumulation of a serous fluid mass, persistent sore throat, enlargement of the lymph nodes in the neck, sub-febrile conditions and worse mood. Peri-tonsillar abscesses and infiltrates and chronic inflammation of the tonsillar tissue refractory to treatment are also common indications. Focal complications are taken into account, for example muscle and joint pain, nephritis or myocarditis. Surgery is also considered in sleep apnoea , when tonsillar hypertrophy and palatal flaccidity impair airway patency; in these situations, surgery improves quality of life. If the hypertrophy is significant and the patient has difficulty swallowing, a recurrent throat infection or bleeding occurs, the ENT surgeon may suggest surgery under general anaesthesia.

Where are the tonsils?

The tonsils are small clusters of lymphatic tissue located within the throat. The most well-known are the palatine tonsils , which are located on either side of the throat, just behind the palatine arches – they can be seen by opening the mouth wide.

What do the tonsils look like?

Healthy tonsils are pink in colour and have a smooth surface, resembling the structure of the oral mucosa. They are usually small and do not cause discomfort. Enlarged or inflamed tonsils become red, swollen and may have plaques or pus on the surface. In cases of chronic inflammation, it is common to see them hypertrophied, which can lead to breathing difficulties, snoring or recurrent throat infections.

Does tonsillectomy affect a child’s immunity?

In fact, tonsils are very necessary until the age of 4-5 years. During this period, we try to remove them only partially, cutting out the diseased part of the overgrown tissue. Excision of part of the hypertrophied tonsils protects against problems with eating, breathing and speech disorders. In older children, their removal does not carry any immune disruption.

What are the effects of tonsillectomy?

Removal of the tonsils reduces recurrent tonsillitis and throat infections and improves breathing and swallowing comfort. However, a sore throat and ears, bleeding, swelling or a change in voice (usually temporary) may occur after the procedure. In children, regrowth of tissue fragments occurs. In the long term, immune functions are taken over by other elements of the immune system.

How long does it take to recover from tonsillectomy?

The most severe discomfort usually lasts for 5 to 10 days, and it takes about 2-3 weeks to recover completely. Healing is faster in children than in adults. A return to work or school is usually possible after 10-14 days, while full wound healing takes up to a month.

What not to do after a tonsillectomy?

Exercise, lifting, smoking and alcohol should be avoided after the procedure. Eating spicy, hot, hard or crunchy foods is not recommended. You also need to avoid fizzy and acidic drinks and be careful when sneezing and coughing to avoid causing bleeding. How much does the throat hurt after tonsil removal?

A sore throat is most troublesome in the first few days after surgery and can radiate to the ears. It usually intensifies over 3-5 days and then gradually weakens. Usually the discomfort subsides within 1-2 weeks, although in adults the pain may last a little longer.
